If a patient loses or misplaces any documents previously issued by the practice—including referrals, care plans, pathology or radiology request forms, or other clinical documents—a replacement may be provided upon request.
Replacement & Administrative Fees
- A $20 reprint fee applies to each lost or misplaced document or when reissuing copies of previously provided paperwork.
- Fees cover administrative costs, including:
- Locating and retrieving records
- Printing, copying, or electronic transmission
- Administrative and clinical time involved
- Re-issuing medical documentation
- Fees must be paid in full at the time the document is issued.
- All fees are non-rebatable and non-refundabe, including through Medicare or private health insurance.
Appointment Requirements
- Issuing a new referral, making changes to an existing referral, or reissuing a prescription requires a consultation appointment with the doctor.
Private Documentation & Assessments
- The completion of private insurance paperwork, pre-employment medical forms, insurance reports, medical record requests, insurance claim forms, and medical assessments incurs an out-of-pocket fee, determined by the complexity and time required to complete the documentation.
- Fees may vary depending on the complexity, time required, and administrative resources involved, and can range from $50 upwards.
Patient Responsibility
Patients are responsible for the safe keeping of all referrals, request forms, and other medical documents once issued.
