If a patient loses or misplaces a pathology or radiology request form that has been issued by the practice, a replacement form may be provided upon request.
Replacement Fee
- A replacement fee will apply for each lost pathology or radiology request form.
- This fee must be paid in full at the time the replacement form is issued.
- The replacement fee is non-rebatable and non-refundable, including through Medicare or private health insurance.
The fee covers administrative time, clinical review, and re-issuing of medical documentation. Patients are responsible for the safe keeping of all referral and request forms once issued.
